Applicants for NBI Clearance who have previously been issued an NBI Clearance from 2014 onwards and have no changes in their personal data (like getting married in the mean time  and changing to their married surnames, for example) can renew their NBI clearance without accomplishing the NBI fingerprint card (blue form).  Please visit the NBI Clearance website for more information.

Otherwise, applicants need to accomplish the NBI fingerprint card (blue form).


REQUIREMENTS FOR THE ISSUANCE OF AN NBI BLUE FORM

  • Two (2) pieces 2×2 sized photographs with white background, taken within the last 3 months (no nametag, no eyeglasses, no accessories, can smile however, teeth should not show in the photo)
  • Photocopy of valid passport
  • Applicable fee: QR100  (cash only - please bring exact amount)


After securing the NBI Blue Form from the Embassy, the applicant sends it to an authorized representative in the Philippines for processing at the NBI Office, Manila. Once the NBI Clearance has been issued, the authorized representative will bring it to the Department of Foreign Affairs (DFA) for the Apostille (authentication).


The NBI Form No. 5 from the Embassy is not the clearance itself, but only an application form for an NBI Clearance.  The applicant must submit the form and other requirements for processing at an NBI office or branch for the issuance of the required NBI Clearance.
