Two (2) pieces 2×2 sized photographs with white background, taken within the last 3 months (no nametag, no eyeglasses, no accessories, can smile however, teeth should not show in the photo)

Applicable fees: QR100 (cash only - please bring exact amount)
After securing the NBI Blue Form from the Embassy, send it to your representative in the Philippines for processing at the NBI Office, Manila. Once the NBI Clearance has been issued, bring it to the Department of Foreign Affairs (DFA) for its authentication (apostille).
REMINDER: Applicants who have previously been issued with NBI clearances from 2014 onwards can now renew their NBI clearance without the need of accomplishing the NBI fingerprint card (blue form) provided that no changes are made to their personal data.
NOTE: The NBI Form No. 5 issued at the Embassy is not the clearance but an application form only to be submitted in the Philippines for processing at NBI office or branch for the issuance of the required NBI Clearance.
IMPORTANT REMINDER: The Philippine Embassy does not issue a police clearance certificate. Further inquiries should be referred to accredited websites of the State of Qatar.

As indicated in the website of Qatar Embassy: Qatar’s Diplomatic and Consulate missions abroad will send the application to the directorate of consular affairs, which in turn will send it to the concerned department (Criminal Evidences and Information Department), and sends the reply to the mission after receiving it.
