REQUIREMENTS FOR REPATRIATION OF HUMAN REMAINS

 

  1. Death Certificate issued by the Supreme Council of Health, Public Health Department of Qatar (original plus 6 copies)

  2. Transfer of Dead Body Certificate issued by the Ministry of Interior of Qatar (original plus 6 copies)             

  3. Notification of Death issued by the Ministry of Public health Central Statistical organization of Qatar (original plus 6 copies)

  4. Photocopy of the deceased’s passport ( 1st and last page) – 6 copies

  5. Photocopy of the passport of the person accompanying the remains ( page with personal data) – 6 copies

  6. Airway bill ( original plus 6 copies)

  7. Embalming and Sealing Certificate issued by the National Health Authority of Qatar ( original plus 6 copies)

  8. Police Report from the Qatar Police Department (in English language) if cause of death is accident or suicide ( original plus 6 copies)

  9. The Embassy may require additional requirements as deemed necessary.

REQUIREMENTS FOR REPATRIATION OF HUMAN REMAINS

  1. Death Certificate issued by the Qatar Supreme Council of Health - Public Health Department - original plus six (6) copies;

  2. Transfer of Dead Body Certificate issued by the Qatar Ministry of Interior - original plus six (6) copies;             

  3. Notification of Death issued by the Qatar Ministry of Health Central Statistical organization of Qatar - original plus six (6) copies;

  4. Deceased passport, 1st and last page - original plus six (6) copies;

  5. Photocopy of the passport of the person accompanying the remains ( page with personal data) - original plus six (6) copies;

  6. Airway bill (indicating details of departure and arrival) - original plus six (6) copies;

  7. Embalming and Sealing Certificate issued by the Qatar National Health Authority - original plus six (6) copies;

  8. Police Report from the Qatar Police Department in English if cause of death is accident or suicide - original plus six (6) copies;

  9. Letter of Acceptance of Human / Cremated Remains - forms to be provided by the Embassy;
  10. Applicable Fees (cash only - please bring exact amount): Report of Death; QR100.00 and Consular Mortuary Certificate: QR100.00 - Total Fees: 200.00

  11. The Embassy may require additional requirements as deemed necessary.

 

The Embassy receives and processes the documents required for the repatriation of the human remains of Filipino national to the Philippines.
