REQUIREMENTS FOR REPATRIATION OF HUMAN REMAINS

 

  1. Death Certificate issued by the Supreme Council of Health, Public Health Department of Qatar (original plus 6 copies)

  2. Transfer of Dead Body Certificate issued by the Ministry of Interior of Qatar (original plus 6 copies)             

  3. Notification of Death issued by the Ministry of Public health Central Statistical organization of Qatar (original plus 6 copies)

  4. Photocopy of the deceased’s passport ( 1st and last page) – 6 copies

  5. Photocopy of the passport of the person accompanying the remains ( page with personal data) – 6 copies

  6. Airway bill ( original plus 6 copies)

  7. Embalming and Sealing Certificate issued by the National Health Authority of Qatar ( original plus 6 copies)

  8. Police Report from the Qatar Police Department (in English language) if cause of death is accident or suicide ( original plus 6 copies)

  9. The Embassy may require additional requirements as deemed necessary.
