- Certificate of No Marriage (CENOMAR) from NSO duly authenticated by the Department of Foreign Affairs (original and 4 copies)
- Marriage Certificate – if married in the Court or Church, it should be translated in English with the stamp of the Ministry of Foreign Affairs in Qatar (original plus 4 copies)
- Birth certificate of both parties: for Filipino citizen, submit original birth certificate from NSO duly authenticated by the Department of Foreign Affairs; and for foreign nationals, submit birth certificate from country of birth.
- Duly accomplished Report of Marriage Form (plus 4 copies)
- Passport copy of both parties (1st page and w/ stamp visa) plus 4 copies
- Four pieces of photos (1×1 of both parties)
- For Divorced, Annulled, or Widowed applicants, submit original and copies of divorce decree, decision regarding annulment, or death certificate of spouse.
- The Embassy may require additional requirements as deemed necessary.
