Requirements for Report of Marriage / LATE REGISTRATION
Certificate of No Marriage (CENOMAR) from NSO duly authenticated by the Department of Foreign Affairs (original and 4 copies)
Marriage Certificate – if married in the Court or Church, it should be translated in English with the stamp of the Ministry of Foreign Affairs in Qatar (original plus 4 copies)
Birth certificate of both parties: for Filipino citizen, submit original birth certificate from NSO duly authenticated by the Department of Foreign Affairs; and for foreign nationals, submit birth certificate from country of birth.
Duly accomplished Report of Marriage Form (plus 4 copies)
Passport copy of both parties (1st page and w/ stamp visa) plus 4 copies
Four pieces of photos (1×1 of both parties)
For Divorced, Annulled, or Widowed applicants, submit original and copies of divorce decree, decision regarding annulment, or death certificate of spouse.
The Embassy may require additional requirements as deemed necessary.
