- Certificate of No Record of Marriage (CENOMAR) issued by the National Statistics Office (NSO) and duly authenticated by the Department of Foreign Affairs – present original and one photocopy
- Birth Certificate issued by NSO and duly authenticated by the Department of Foreign Affairs – present original and one photocopy
- One (1) photocopy each of the passport of the contracting parties with their personal details and signature
- Affidavit of Intent to Marry
- Philippine address of the Filipino party
