Submit the following documents (original plus 5 photocopies each)

1. Report of Birth Form (FA Form No. 40)

a. Entries must be typewritten or printed legibly

b. Entries must be at the time of birth of the child

c. Must contain the printed names and signatures of witnesses ( disinterested persons)

2.  Negative Certification of Birth Record from the National Statistics Office

3.  Birth Certificate with English translation

4.  First passport of the child or travel document (photocopies of data pages) 

(in case of unavailability, execute affidavit of loss/explanation plus copy of present passport)

5.  Authenticated marriage certificate of parents issued by NSO

6. Passport of parents used at the time of birth of the child (photocopies of data pages) in case of unavailability, execute affidavit of loss/explanation plus copy of present passport

7.  Five recent passport size (4.5x 3.5 cm) photos of the child.

 

REPORT OF BIRTH AND PASSPORT ISSUANCE

  1. Duly accomplished Report of Birth Form

  2. Duly Accomplished Passport Application Form

  

DOCUMENTARY REQUIREMENTS:

Original and five (5) copies of the following:

  1. Qatar-issued Birth Certificate (white colored);

  2. Qatar-issued Birth Certificate (yellow colored);

  3. Marriage Certificate or Report of Marriage issued by the Philippine Statistics Authority (PSA) duly authenticated by DFA-PH (Apostilled); and

  4. Valid passport of both parents (copy of data page only)

 

For births not registered within one (1) year

  1. Affidavit of delayed registration;

  2. Affidavit of two disinterested persons; and

  3. Four (4) recent passport-size photos of child

     
Note: The Embassy may require additional requirements as deemed necessary.
