PASSPORT

A. Passport Renewal Requirements:

  1. Personal Appearance
  2. Duly-accomplished passport application form (Adult or Minor form)
  3. Original and photocopy of data page of most recent passport
  4. Processing Fee of QR240 (cash only)
  5. (For minor applications only) Copy of Qatar ID (QID) or passport of the accompanying parent or legal guardian

B. Passport Requirements for Newborn

     Follow requirements for Report of Birth (see under Consular Services-Civil Registry-Birth)

Note: Passport processing time is 30-45 days (average)

C. Additional Requirements:

   1. Replacement of Lost Passports:

  1. Original and photocopy of birth certificate from the Philippine Statistics Authority (PSA) and authenticated (or with red ribbon) by DFA;
  2. For married women, original and photocopy of marriage certificate from Philippine Statistics Authority (PSA) and authenticated (or with red ribbon) by DFA;
  3. Police report (with English translation);
  4. Publication of Notice of Loss in an English newspaper;
  5. Photocopy of any Philippine-issued ID, such as driver's license, voter's ID, Professional Regulatory Commission (PRC) ID, school/college ID, etc.
  6. Photocopy of data page of lost passport, if available;
  7. Duly accomplished passport application form (Adult or Minor form);
  8. Affidavit of Loss;
  9. Personal Appearance;
  10. Payment (see below)

 Fees for Lost Passport (cash only):

  • QR100 - for Affidavit of Loss;
  • QR100 - for verification, if passport was not issued at Philippine Embassy in Doha
  • QR600 - for replacement

Please take note that there will be a 15-day clearing period prior to the processing of application for replacement of lost valid passport.    

 

   2. Renewal of Green/MRRP or Maroon/Machine Readable Passports:

  1. Original and photocopy of birth certificate from the Philippine Statistics Authority (PSA) and authenticated (or with red ribbon) by DFA
  2. For married women, original and photocopy of marriage certificate from Philippine Statistics Authority (PSA) and authenticated (or with red ribbon) by DFA

   3. Change of Surname (For married women):

  1. Original and photocopy of marriage certificate from Philippine Statistics Authority (PSA)  and authenticated by DFA (with red ribbon or authentication); or
  2. Original and photocopy of Report of Marriage issued by the Embassy

   4. Reversion to maiden name (For married women):

  1. Original and photocopy of birth certificate from the Philippine Statistics Authority (PSA) and authenticated by DFA (with red ribbon);
  2. If widowed, original and photocopy of duly-authenticated Death Certificate of the deceased husband;
  3. If marriage is annulled, original and certified true copy of duly-authenticated annotated marriage contract from the Philippine Statistics Authority (PSA) and authenticated by DFA (with red ribbon); original and certified true copy of Certificate of Finality and Court Order;
  4. If divorced, certified true copy (CTC) of the Divorce Decree duly-authenticated by the Philippine Embassy or Consulate where the divorce was obtained or by the concerned foreign diplomatic or consular mission in the Philippines, or duly-authenticated or certified true copy of Philippine court recognition of foreign divorce decree.

   5. For converts who would like to use their Muslim name:

  1. Original and photocopy or duly authenticated Birth certificate bearing the Muslim name
  2. Original and photocopy of duly authenticated National Commission on Muslim Filipinos (NCMF) or Office on Muslim Affairs (OMA) Certificate of Conversion;

    6. If applicant has reacquired his or her Philippine citizenship:

  1. Submit original and photocopy of Identification Certification and Oath of Allegiance issued by the Philippine Embassy or Consulate with jurisdiction or by the Bureau of Immigration (BI).
